What is SAP? (Student Assistance Program)
The Logan Elementary Student Assistance Program (SAP) consists of a multidisciplinary team of education professionals. The goal of the team is to remove barriers to learning that impede student success. Through a referral process, the SAP team identifies needs (mental health concerns, substance use, etc) and then mobilizes school and community resources to address these challenges.
Referral
Staff members, parents, and students can make SAP referrals when a need or concern arises. The SAP team may then contact the parent for permission to proceed with the SAP process.
Team Planning
The SAP Team collects and reviews information regarding the referred student, including academic and behavioral data. Meetings/discussions with parents and students may take place to gain more insight into the matters impeding student achievement. Through a collaborative process, a case manager is assigned and a plan is developed that utilizes school and/or community resources.
Intervention & Recommendations
The developed plan is then implemented. Depending on the level of need, the SAP team may recommend the utilization of community-based mental health resources (Turtle Creek Valley Community Services). This process begins with parent consent to a behavioral health screening conducted by the TCV SAP liaison, who will then make recommendations regarding the assessment.
Support & Follow-Up
With interventions in place, students will continue to be monitored for improvement and additional support.
