Incoming Kindergartners are required to have the following records submitted to be fully registered for the upcoming school year:
- Completed Vaccination record or Immunization exemption form
- Completed health physical dated before 8/24 (Please make sure it is a physical and not the Child Health Report)
- Completed dental exam dated before 8/24
- Should your student require medication during the school day, please have the physician fill out the Medication Administration form.
NOTE: Over the counter medications and prescription medications require a physician's order to be administered in school.
